Student Application
If you are a student wanting to go on a VSM trip, please fill out the following application. If you are wanting to go on a VSM Basketball or Baseball trip, DO NOT fill out this application, but rather scroll down and fill out the VSM Basketball Application form or the VSM Baseball Application form.

Basketball Team Application
If you want to apply for a VSM Basketball Team, please complete the following form. To go on a VSM Basketball Team you must meet the following criteria:
1. Must be male.
2. Must be at least 16.
3. Must know how to play basketball.

If you want to apply for a VSM Baseball Team, please complete the following form. To go on a VSM Baseball Team you must meet the following criteria:

VSM Leader Application
If you would like to lead a VSM team, please fill out the following application. To lead a VSM you must be at least 21 years old.

Boot Camp Application
If you would like to attend Boot Camp to see what VSM is all about, please fill out the following registration form. Boot Camp is at Daniel Springs Camp in Gary, TX and it is held the 2nd week in June each year. The cost of Boot Camp is $100 and you must be at least 14 at the time of camp to attend.
VSM team members are required to attend boot camp and are automatically enrolled and are not required to fillout a Boot Camp registration form.

Recommendation Form
All VSM applications are required to have a pastor or youth pastor fill out a recommendation form. Please have your pastor or youth pastor fill out the following recommendation form.
